Ok so there are a few contributions for the States, UK and Australia, so I thought I'd start up for any potential Asian visitors. When I get around to it I might do guides for Singapore, Hong Kong and Bali...but no promises. This guide is in no way definitive and can be edited/changed at any given time.
Roppongi
The nightlife capital of the capital, although Roppongi is pretty well known for also being the "seedy hangout for all the foreigners who can't pick up elsewhere" it's still a safe bet to get by if Japanese isn't your native (or even second) tounge, because virtually everyone in this area will speak English.
Vanilla - A huge club on 2 different floors, contains dance floors for Hip Hop and Psychedelic Trance (2nd floor) and Top 40's/Disco/Mainstream Trance (3rd floor), as well as numerous chill out spots and VIP lounges. Entry is 1,000 yen (about $10USD) for foreigners which includes 2 drink tickets (entry is more expensive for Japanese guys...they really rely on the "chick pulling power" of foreign guys!). Drinks cost on average 800 yen. A GREAT guy to girl ratio here, and the girls go out of their way to REALLY dress up. There are dance podiums on the main dance floor which guys are NOT allowed to be on at any time. Only opens on Friday and Saturday nights. Is dead up until around 11pm, when it really gets packed....usually shuts down around 6am. Probably holds a good 1,000 - 2,000 clubbers at any given time. Unfortunately music can be rather loud. From Roppongi crossing stand in front of Almond, get to the other side of the road and walk towards Roppongi Hills. Vanilla is down a narrow alleyway about half a block from Roppongi Crossing. HB 9
Yellow - Was really famous imediately prior to the burst of the bubble, and has never really recovered. Is really expensive to get in, with cover charge usually around the 3,000 yen mark ($30USD) which will normally only get you a free drink ticket. Having said that the HB rating here is REALLY high. Also tends to get in internationally renouwned DJs on a regular basis, so if this is your thing then you probably won't mind shelling out the cash. Drinks cost average about 1,000 yen a drink. On the opposite side of the road from Vanilla and walk in the other direction from Almond/Roppongi Crossing. Also down a quiet side street, it can be really tough to find because there are no signs, just 2 giant double doors usually with one doorman. Fits about 1,000 clubbers. HB 9.5
Velfarre - Another mega club within 2 blocks of the other two, Velfarre has the unusual distinction of closing at 1am, which is after the last train has run, meaning this is only a good spot to "start" the night. Features big name DJs on a pretty regular basis, and HB quality here isn't bad at all. Unfortunately I can't recall the cover charge or drinks cost! HB 8
Sports Cafe - More of a bar than a club, this place is a common hangout for the "off duty" strippers and hostesses (which means it gets really busy in the very early hours of the morning - 3am onwards). Most of the girls here will be foreign entertainers from the Eastern Block and Russia so you have to tread very carefully - they are professionals. Probably boasts the most gorgeous foreign women in one place in Tokyo, if you get bored of the pretty Japanese girls. A lot of these girls are waaaaay too use to all the attention so some serious negging is nessisary. Only been successful with 2 strippers/hostesses - a Singaporean and a Canadian. Very tough game. Free entrance, drinks cost on average 800 yen. Can be found directly behind Private Eyes strip club or just around the corner from Velfarre. HB 7.5
Gas Panic - Part of a chain, there is one in most cities. Stay away from them unless you want to try out your serious AMOG skills, these are the most famous places for off-base US servicemen. The girls that go here generally only go to get laid, which might tell you about their qualities. Still nice to go to if you want to pick up your self confidence first before trying on your game! Free entrance and drinks cost on average 600 yen. Is across the road from Tony Romas down a side street. HB 6

Roppongi is definately amazing, Gas Panic and Sports Cafe are the only ones I remember specifically, I have no idea how I got back to my hotel after going there!
